Analysis of Autonomic Nervous System Activity in Trapezius Myalgia by Analysis of the Heart Rate Variability(HRV)

Abstract
Objectives: The pathophysiological link between stress and trapezius myalgia remains a puzzle. Stress-induced shoulder and neck pain are not necessarily associated with elevated trapezius muscle activity. It has been reported that sympathetic vasoconstriction is one of the pain-initiating mechanisms which are related to stress and not mediated through muscle activity. The study was designed to investigate the sympathetic nerve activity in trapezius myalgia, using the power spectrum analysis(PSA) of the heart rate variability(HRV)

Method: 46 outpatients with Trapezius myalgia who visited the Dept. of Oriental Rehabilitation Medicine in Conmaul Oriental Hospital from March, 2003 to April, 2004 were selected as a study group, and 39 healthy individuals as a control group. Total power(TP), Low Frequency(LF), High Frequency(HF), and LF/HF ratio were used as the indices of HRV. T-test was used to compare the HRV of Case and Control.

Results: LF/HF ratio was significantly higher in trapezius myalgia. (P<0.05) But, there was no significant difference between both groups on TP, LF, HF.

Conclusion: The results suggest that Trapezius myalgia is correlated with the sympathetic nerve activity.
